Symptoms I see with this.

Caching the checkout page
Full-page caching that is not excluding cart, checkout and account pages produces bizarre, intermittent failures — including customers seeing each other's carts. This is the first thing I check.
Gateway webhook not being received
Payments complete and the order never updates because the callback fails or is rejected. The money exists; the order does not know it.
JavaScript error blocking submit
A conflict between a plugin and the checkout script stops the form submitting. Often browser-specific, which is why it looks random.
Plugin conflict on checkout fields
A field-customisation or shipping plugin producing invalid data that fails validation silently, so the customer sees nothing happen.

Those are the most diagnosable cases, because the pattern tells you a lot — a specific browser points at JavaScript, a specific country at a gateway rule, a specific product at plugin logic.
